This form is required for all new members. It must be notarized (there is a working notary in the University Center) and is due the first day of training so that we can photocopy it for our records and then provide it to the Office of Emergency Management (OEM) so that the individual is allowed on the USAR site.
Make sure you bring it with you to the first training session!
